Children's health services include:

  • Health supervision
  • Health promotion education
  • Well child exams
  • School, camp, and sports physicals
  • Growth and development screening and education
  • Illness diagnosis and treatment of minor illnesses
  • Education about chronic conditions
  • Management of stable chronic conditions
  • Immunizations
  • Laboratory testing
  • Hoosier Healthwise Enrollment Center

Adult health services include:

  • Yearly wellness physical exams (including DOT/CDL and work physicals)
  • Blood pressure screening
  • Vision and hearing screening
  • Health promotion and health maintenance education
  • Smoking cessation programs
  • Nutrition and exercise counseling
  • Diagnosis and treatment of minor illnesses
  • Management of chronic stable conditions
  • Blood sugar checks for diabetes
  • Cholesterol and triglyceride screening
  • Laboratory testing

Women's health services include:

  • Yearly health assessment and physical exams
  • Pap smears (may be free for women who are eligible)
  • Mammogram referral (may be free for women who are eligible)
  • Free pregnancy testing
  • Prenatal care
  • Smoking cessation programs
  • Nutrition and exercise counseling
  • Stress management

Men's health services include:

  • Yearly physical exams
  • Prostate screening (including free PSA for men age 50-70 or earlier if positive family history)
  • Smoking cessation programs
  • Nutrition and exercise counseling
  • Stress management
